Nitric oxide is a molecule your body makes naturally in the lining of your blood vessels. Its main job is to signal blood vessels to relax and widen, a process called vasodilation, which is why it is central to cardiovascular health and healthy blood pressure.
What Nitric Oxide Actually Does in Your Blood Vessels
The inside lining of every blood vessel, called the endothelium, produces nitric oxide continuously. When endothelial function is working well, blood vessels can widen and narrow in response to what your body needs. This flexibility is a major factor in maintaining healthy blood pressure. As we age, natural nitric oxide production tends to decline, which is one reason vascular health becomes a bigger focus in your 40s and beyond.
What the Clinical Research Shows
The strongest pooled results have been reported in adults with hypertension. A 2024 systematic review and meta-analysis of 11 randomized trials involving 349 patients with clinical hypertension found that nitrate-rich beetroot juice reduced clinic systolic blood pressure by an average of 5.31 mmHg compared with control treatments. The 95% confidence interval ranged from 3.16 to 7.46 mmHg. The authors rated the certainty of this evidence as low and found no significant effect on diastolic blood pressure or 24-hour blood pressure.
A 2022 meta-analysis of seven randomized trials involving 218 adults with hypertension reported a 4.95 mmHg reduction in systolic blood pressure compared with control treatments. That evidence was rated moderate by GRADE. The analysis did not find a significant diastolic effect.
Across broader adult populations, a 2017 meta-analysis of 22 randomized trials found average reductions of 3.55 mmHg in systolic pressure and 1.32 mmHg in diastolic pressure with beetroot juice. Benefits were larger in studies lasting at least 14 days and with higher nitrate doses, supporting the importance of consistent daily intake rather than occasional use.
The 2024 hypertension analysis studied daily nitrate intakes of 200 to 800 mg and found no sign that the blood pressure response weakened through tolerance. A larger 2025 dose-response meta-analysis covering 75 randomized trials and 1,823 participants also reported improvements in vascular measures, including flow-mediated dilation and pulse wave velocity.
A separate crossover trial using nitrate-rich beetroot juice found larger reductions, around 7 mmHg systolic and 6 mmHg diastolic compared with placebo, though the researchers also noted meaningful variability between individuals. Not everyone responds the same way. A review of 41 separate trials found about half showed a statistically significant reduction in systolic blood pressure, while fewer showed an effect on diastolic pressure.
The evidence supports a real and measurable effect from dietary nitrate, with more pronounced pooled systolic reductions in adults with hypertension than in broader or healthy populations. Results remain more consistent for systolic pressure than for diastolic pressure, and the certainty of evidence varies across analyses.
Where Dietary Nitrate Comes From
Beets, leafy greens like spinach and arugula, and other vegetables are the richest natural dietary sources of inorganic nitrate. Once consumed, nitrate converts to nitrite and then to nitric oxide in the body, which is the same pathway targeted by beetroot-based supplements.
The Bottom Line
Nitrate-rich foods and supplements have peer-reviewed evidence supporting nitric oxide production, vascular function, and healthy blood pressure. Consistent daily intake over two weeks or longer produced stronger results in pooled research. Dietary nitrate should remain part of a broader approach that includes exercise, sleep, balanced nutrition, and standard cardiovascular care.
